Delivered as a digital document with contact information and service descriptions.
Personalized Care Plan
We develop a personalized care plan based on the needs assessment, outlining specific services and goals. This includes coordinating appointments, managing communication between providers, and monitoring service delivery. Includes monthly check-in calls and quarterly care plan reviews.
Advocacy Care Management
A comprehensive, hands-on service providing ongoing advocacy, care coordination, and support. We act as a primary point of contact for all care-related matters, attend doctor’s appointments (virtual or in-person), manage communication with healthcare providers, monitor care quality, and advocate for the client’s rights and preferences. Includes weekly check-in calls, regular home visits (if applicable), and assistance with navigating insurance/Medicare complexities.
