Ongoing support is provided through a monthly check-in call, allowing us to address questions and offer guidance. Proactive Support

For families actively managing ongoing care, our Proactive Support level offers a more in-depth partnership. This includes all the benefits of Essential Guidance, plus dedicated appointment companionship for up to two hours each month. We’ll attend medical appointments with you, taking notes, asking clarifying questions, and providing emotional support. We also facilitate quarterly medication reconciliation with your healthcare provider, ensuring accuracy and effectiveness. Beyond this, we offer assistance with organizing and maintaining important medical records, and provide unlimited phone and email support for ongoing questions and concerns. This level is ideal for those seeking hands-on assistance and proactive management of their healthcare journey.

Comprehensive Advocacy

Our most comprehensive level of care is designed for families facing complex healthcare challenges. It includes all the benefits of the previous levels, plus a dedicated Care Coordinator who serves as your single point of contact. We collaborate with healthcare providers to develop and implement a personalized care plan, and actively advocate on your behalf with both providers and insurance companies. We facilitate regular care team meetings to ensure everyone is on the same page, and provide crisis intervention support when urgent situations arise.
